Could your eyes hold the secret to your health? Scientists have developed a cutting-edge AI that uses a simple photo of your retina to predict your true 'retinal age' – and potentially flag major disease risks like diabetes and heart problems.

This incredible new tech could offer a super easy way to spot potential health issues without extra tests.

Experts at Tohoku University have created an artificial intelligence (AI) model that assesses a person's 'retinal age' from one single photograph of the back of the eye. This 'retinal age' reflects biological ageing.

It turns out, the gap between your real age and this estimated retinal age might be a big red flag for diseases.

Professor Toru Nakazawa, who led the research, explained: "Fundus images are non-invasive photos of the eye taken as part of regular health check-ups – so no additional work is needed. Our model would be a nearly frictionless addition to a clinician's typical workflow."

How the AI works its magic

This clever AI was trained using over 50,000 eye photos from healthy adults. It then validated itself on nearly 7,300 more images.

The system got pretty good at guessing actual age, with an average error of about three years.

The 'retinal age gap'

Researchers then looked at the 'retinal age gap' – that's the difference between the AI's predicted retinal age and a person's actual age.

They found this gap was significantly larger in people who had diabetes, heart disease, or a history of stroke. This means their eyes appeared older than their actual age.

So, an 'older looking' retina could point to hidden health concerns.

What's next for this breakthrough?

While the findings are exciting, the team notes that more studies are needed to confirm if the retinal age gap can actually predict future disease onset.

Professor Nakazawa is already on it. "We are already planning a study that follows a cohort of over 10,000 individuals with continuous 3-year follow-up to examine whether retinal age-related signals are associated with the future development of cardiovascular and other systemic diseases."

The tool is designed to be a helpful screening aid in the future. It could reveal patients who might need more health assessments or personalised prevention strategies.

The findings were published in Communications Medicine on April 8, 2026, with external validation support from University College London.
